By submitting this form:

1. You acknowledge that you aware that you are providing consent for a Nationally Coordinated Criminal History Check to be conducted using all Personal Information provided in this form and in supplied identity documents.
2. You acknowledge that Crime Check Australia is collecting information in this form to provide to Australian Criminal Intelligence Commission (ACIC) (an Agency of the Commonwealth of Australia) and police agencies;
3. You give consent to Police Information relating to you, being disclosed in accordance with the purpose identified in this form, and applicable legislation and information release policies (including spent convictions legislation described in Australian Government and state or territory legislation).
4. You acknowledge and consent to your personal information being disclosed to police agencies and used for general law enforcement purposes including the investigation of any outstanding criminal offences, including those purposes set out in the Australian Crime Commission Act 2002 (Cth).
5. You have fully and correctly completed this form, the personal information you have provided in it relates to you, contains your full name and all names previously used by you.
6. You acknowledge that the provision of false or misleading information on this form is a serious offence.
7. You acknowledge that any information sent, by mail or electronically, in relation to this form, including any identity documents, is sent at your own risk and you are aware of the consequences of these methods of lodgment.
8. You give consent to Crime Check Australia to verify the identity documents that you supplied using an Authoritative Source, including, where necessary, searching, retrieving and storing Visa Entitlement Verifications Online – commonly known as VEVO Checks-.
9. You give consent to Crime Check Australia to provide the document identifiers of the documents that you supplied to the ACIC for the purpose of the ACIC using the DVS to verify your identity documents.
10. You give consent to the ACIC using the document identifiers of the documents that you supplied to verify your identity documents using the DVS.
11. You give consent to the ACIC and police agencies using and disclosing your Personal Information that you have provided in this form, Personal Information contained in my supplied identity documents and all other available Personal Information relating to you, to conduct a Nationally Coordinated Criminal History Check.
12. You give consent to the ACIC disclosing the Police Information sourced from the police agencies to other approved bodies and Crime Check Australia.
13. You give consent to Crime Check Australia to disclose your Personal Information (including Police Information) to the Crime Check Australia’s contractors for the purpose of conducting a Nationally Coordinated Criminal History Check.
14. You give consent to Crime Check Australia to disclose to the Customer and Third Parties detailed in this form, your Personal Information (including Police Information) to assess your suitability for the purpose identified in this form and as required by Australian law.
15. You acknowledge that you have read and understand the information on this form.
